There are nearly 15,000 Skilled Nursing Facilities (SNFs)—also known as nursing homes—across the U.S. Each month, some are removed from the CMS database.

While many removals are due to facility closures, others may result from consolidations or changes in Medicare/Medicaid designation that require a new entity to be created.

At StarPRO, we focus on operational facilities, but we believe it’s equally important to track homes that have been deactivated.

Below, you’ll find our list of facilities that appear to have closed, organized by the month of deactivation. Note: CMS does not publish the reason for deactivation. We encourage you to use this list as a starting point for your own research.
