There are nearly 15,000 Skilled Nursing Facilities (SNFs), also called Nursing Homes in the U.S. Each month, several of these homes are removed from the CMS database.
Often, this is due to the nursing home actually closing. But sometimes the home is being consolidated into another organization or has experienced a change in their Medicare/Medicaid designation, requiring a new entity to be created.
At StarPRO, our primary focus is on homes that are operational, but we still think it’s important to list out homes that appear to have closed down. Below is that list, organized by the month in which the home was deactivated. CMS does not publish a reason for deactivating a home, so use this list to perform your own research!
